While our practice has grown dramatically over the past few years, we continue to offer a warm and welcoming environment with a helpful, cheerful and pleasant staff, several comfortable waiting rooms, a coffee bar, a lending library of books and medically-oriented videotapes and an extensive array of educational materials to take home.

In addition to individual therapy for adults and selected adolescents and children, we offer couples therapy for adults as well as a number of ongoing psychotherapy groups.

Mt. Washington Psychiatric Services is a group of clinicians and staff who provide mental health outpatient services to adult and older adolescent patients. We offer a therapeutic environment using the techniques of psychotherapy and the methods of modern pscyhotropic medications.

 

 

Privacy and Confidentiality Policy

 

We recognize how important it is to maintain and protect your privacy. Your patient records are protected from disclosure under both state and federal laws relating to mental health services. Conversations and test results are held in strict confidence unless otherwise provided for by state or federal regulations such as: You are a danger to yourself or to others, or a child is endangered.

If your provider needs to consult with someone regarding your treatment, you will be asked to sign a release form that will clearly identify the information to be exchanged, the parties involved in the exchange, and the reason for the communication. The original release form will become part of your permanent record.

Except as provided by law, we will not disclose information about your treatment, or even the fact that you have been seen at Mt. Washington Psychiatric Services, to anyone without your written permission.

All patient records are secured in a locked storage area at Mt. Washington facility.

Patient Rights and Patient Responsibilities

 

Patient Rights

As a patient at Mt. Washington Psychiatric Services you are entitled to:

  • Be an active participant in treatment decisions during your care.
  • Be treated at all times with dignity and respect. Every member of our staff will help you in any way he or she can.
  • Voice a complaint or appeal a decision about care provided to you.
  • Inspect your own records with the condition that your provider may temporarily remove portions of the records if they believe the information is detrimental to your therapy. All requests for this inspection are to be made in writing to the Practice Administrator, and specific arrangements for viewing records will be made on a case-by-case basis.

 

Patient Responsibilities

 

As a patient being treated at Mt. Washington Psychiatric Services, you have a responsibility to:

  • Give your doctor, therapist and our staff the information they need to provide you with appropriate care.
  • Follow your doctor's and therapist's recommended plans and instructions for care. Participate in the treatment process through a focus on goal-setting and by the development of a mutally agreed-upon treatment plan.
  • Inform the staff of any changes in your health insurance coverage, home address or telephone number.
  • Keep scheduled appointments and be sure to give us 24-hour advance notice if you must cancel an appointment.
  • Assist us in obtaining any necessary pre-authorization for treatment from your insurance carrier.
  • Plan ahead for prescription usage so you don't run out. Use our voicemail system for requesting refills
  • Make payment of all insurance co-payments and deductibles at the time of service
  • You are ultimately responsible for the cost of your treatment. We will be glad to assist you in obtaining proper payment from your insurance carrier.

Working together we will strive to achieve a reduction in symptoms, relief from illness, improved functioning, and increased insight
